Abstract

Objectives: To determine the prediction factors to clinical deterioration of discharge ICU patients in Department of Neurology and Neuro Intensive Care, Viet Duc University Hospital.

Methods: Descriptive cross-sectional study was performed on 95 ICU patients. The National Early Warning Score (NEWS) is used to identify patients at risk of clinical deterioration after discharge from Intensive Care Unit.

Results: The prevalence of patients developing clinical deterioration was 15,8%. Age, intensive care unit length of stay, types of ICU discharge, NEWS score, and using vasopressor at the time of discharging ICU were significantly associated with the occurrence of clinical deterioration; in which age, NEWS score, and using vasopressor at the time of discharging ICU were statistically significant prediction to early clinical deterioration.

Conclusion: ICU discharge is strongly associated with several demographic characteristics, therefore associated with poor prognosis. Early clinical deterioration is statistically significant association with length of stay in the ICU and the consequences is increasing costs and hospital-acquired complications. Therefore, in order to improve patient outcomes and limit the occurrence of early clinical deterioration, it is necessary to train medical staffs, especially ICU nurses on using the NEWS scale so that they are able to detect high-risk patients (older, high NEWS score, and using vasopressor at the time discharging ICU) will help reduce the percentage of early clinical deterioration
